Below you will find our New Patient Forms. If you have access to a printer, go ahead and print them, fill them out then, bring them with you to your first appointment.
If you can not print them out, no worries, come 10 minutes early to your first appointment and we will have a copy for you to fill out in office.

We accept Visa, Mastercard, American Express, Discover, Check, Cash and Apple Pay.
We also have a convenient online portal that you can easily pay your bill through.
We do offer in house payment plans as well. When your proposed treatment has been diagnosed and recorded into your records, we can work up a payment plan accordingly. Payment plans are created upon request. Payment plan guidelines are subject to change, your final agreement will be presented at your appointment.
